We are seeking an experienced and motivated Registered Nurse with experience across hospital and community settings, and a strong interest in prosthetics, orthotics and rehabilitation. This role is an integral part of our national rehabilitation team, supporting people to achieve the best possible functional outcomes, independence and quality of life.

Job Description

About the role

This role plays a key coordination and clinical support function, ensuring patients entering Peke Waihanga services are assessed, supported and connected to the right services at the right time. Working closely with an interdisciplinary team, you will provide holistic nursing care, health promotion and care coordination across the patient journey.

Key responsibilities include:

Patient coordination and care

  • Coordinate and facilitate new patients entering the service
  • Provide holistic nursing assessment, treatment and ongoing care
  • Review referrals and liaise with patients, whānau and health professionals to ensure patient‑centred scheduling
  • Triage incoming patient calls and ensure timely, appropriate intervention

Clinical support and health management

  • Support patient management of diabetes, dialysis, pain and cardiovascular conditions
  • Provide wound care during prosthetic, orthotic and physiotherapy appointments
  • Deliver health education and health promotion with a focus on secondary prevention
  • Support patients to identify realistic, individual goals that consider health and lifestyle needs

Interdisciplinary collaboration

  • Actively participate in interdisciplinary team meetings and patient clinics
  • Contribute to prosthetic and orthotic prescriptions and associated documentation, including high‑technology applications
  • Ensure accurate and timely clinical documentation
  • Build and maintain strong relationships with Te Whatu Ora services and other health providers

Service development and outreach

  • Make timely referrals to support a coordinated, ‘wraparound’ service for patients
  • Support access to services closer to home by attending and promoting regional clinics
  • Contribute to the orthotics care space, including foot reviews, wound care advice and strengthening links across tertiary and primary care
  • Help build the profile of Peke Waihanga within the wider health community

About you

You are a registered nurse who enjoys working in a collaborative, interdisciplinary environment and is motivated by making a meaningful difference in people’s lives.

You will bring:

  • Experience working across hospital and/or community health settings
  • Strong clinical assessment and care coordination skills
  • Excellent communication skills and the ability to build trusted relationships
  • Experience or interest in rehabilitation, vascular, diabetes or orthopaedic care
  • A strong interest in prosthetics and orthotics
  • Wound care experience beneficial
